Create a form and other chnages

Hello, i am looking for a freelancer that can build a form that will interact with sql database to create templates for the database headers for different devices. then when a new device is created on our visualization we can select a template that has been created. We want this built due to the fact we are dealing with multiple different devices and the same model of device can have different modules attached. We can currently handle the one format but we would like to expand our service to include other devices and other configurations we currently/ will have in the future. Along with this there is a few minor changes that we need done to the current website.
