Design a 'dating card' for singles event company

We need someone to design a 'dating card' for a singles event company. When our clients arrive at our events we will hand them a 'dating card' this can then be filled out during the event and results sent into us so we can match people up. This will need to be on a single sided A5 piece of card. We need our company logo on there and also a table with 5 columns under the heading Name, Date, Friend, No and Comments. The Date, Friend, and No columns will just be a tick or cross column so don't need to be as big. We will have a maximum of 20 names on there and need these name columns to have consecutive numbers on them and room for the people to write names down. The other columns can be left blank. We need to have the following paragraph somewhere on the design 'Go to the results page on our website at [login to view URL] to enter your results. Remember you only have 7 days to get your results in!' We need this in the Raleway font type. Our website is published but not finished so is free to look at to get an idea of the colour palette we have used. Thanks
