Business Card design from Logo

I have a logo. I would like to create a business card template from it.
I will provide the logo and you will create a template file which I can modify and upload to one of the many online business card printers.
The template will have to be in CMYK, 300 dpi, and the business card will stand vertical. Deliverables will be in EPS, PDF, and AI formats. You will also include any fonts you use. I am on Windows.
I will have Name, position, address, phone numbers, email address and company logo on the front. Web URL and the company colors on the back.
The card will be printed on very heavy paper stock and be matte.
To be chosen for this project, please submit samples of previous business card work. Don't send me webpage portfolios because it doesn't show me what your skills are with business cards.
I am expecting at least 3-5 sample designs from which I choose one to modify until it is perfect.
