Review each tab and follow the yellow highlighted instructions to select from existing fields or create your own custom fields.
Each field type has two drop downs associated with it: "Data type" and "Required (Yes/no)." Click on the field to make your selection.
If you choose the "Drop Down" data type, type in your entries in the space provided at the bottom of each sheet.
Submit the completed Setup Sheet to your Implementation Representative for review.
Once received the Implementation Representative will setup the fields as recorded on this sheet and schedule a call with the Governance Board to confirm.
