Sign Up Sheet for Covid Testing Times at an Event with Google Forms/Sheets

I have 410 attendees at an event and they will all need to sign up for a covid testing time on one of 2 dates. There will be testing times every 15 minutes from 11:30am - the last testing time at 6:45pm on each of the 2 dates. Each testing time can have a maximum of 12 people. I need to have a form created where they can add their name, email address, and birthdate, and select one of the 15 minute testing times, with a maximum of 12 people allowed to select each testing time. And I would like this to be organized in time order on a google sheet.
