I need a straightforward application form built in AppSheet to capture product / service requests. The first release should focus solely on customer information—name, email, phone, and any other standard contact details—so clients can raise a request quickly without extra fields getting in the way. I’ll give you collaborator access to my AppSheet account (or you can transfer ownership on completion). Please create the underlying Google Sheet, configure the form, apply a clean layout that works on both mobile and desktop, and turn on basic email confirmation so I get notified whenever a new request arrives. Deliverables • Ready-to-use AppSheet app shared to my account • Linked spreadsheet with clearly labeled columns • One-page note explaining how I can add more fields later I’ll consider the job done once I can open the form, submit a test entry, and see the data appear immediately in the sheet.
