Hello! Respected Client, It's simple project for me and I can integrate mailchimp opt-in form on shopify website. You will have to provide shopify credentials so I can make changes in template. For the last part, you need custom email with your own branding material. This should be hosted on different server. Custom email template creation and email script will cost you $25 AUD additional.
