Develop a complex, persistent web survey that can store and modify past responses

Hi, I'm director of a small academic office. We run an annual data collection survey of about 30 questions for about ~100 cities each year since 1990. Our institution supports Qualtrics XM and Google Forms.
We need a Qualtrics wizard or Google Forms ninja who can ideally use one of these tools to develop an online survey, which can import and export data stored in timestamped flat files (csv).
Or, make a case that a custom web form in your language of choice is easier and more maintainable! Think of this is an elementary school version of a custom web form with SQL backend that relies on a survey and csv files instead.
Requirements:
1-- able to password protect the survey (either one password, or unique passwords)
2-- able to identify individual respondents (either a unique password, or URL, or selection from a drop-down menu to identify each respondent on behalf of their city)
3-- able to save and reload partly completed forms for each respondent
4-- able to import and edit previous years' responses for each respondent
5-- able to display help/tooltips for assistance completing form fields
6-- able to run data validation on responses
7-- able to audit/track edits made over time by various authors. A short script to backup as changes are made. Or even better, a short script to push changes to github so that modifications to the csvs are tracked there.
