Crystal Report customization for MRP software

We have a crystal report that has been customized for a particular MRP, and is supposed to print QR Code data on it. We have the crystal report file, and we have a printout of what its supposed to look like, but not the data source names. It will be mainly either repairing this report to function correctly, or perhaps taking the default purchase order form and re-customizing it. We would like the report to be editable in Crystal 2011 if possible, as that is what we have available to us, but it is not a requirement. Documentation of the data source mappings and how to make future changes to the report is required, as these are controlled documents in our quality system. Screenshots are a sample of how it used to work as well as what the default report looked like before customization.
