I am wanting someone to build a simple care management system for a small charitable organisation.
I am needing someone who is able to take a number of assessments and forms (currently in either Word or PDF), make them electronic and be able to produce a summary document/report that can be viewed online and printed. We also need the ability to scan and attach documents to the patients individual file.
The system needs to be set up so patients can be set up and then allocated forms and they can be stored electronically and securely and can be completed on PC or deployed on our website with login and password.
We are a small operator so do not need the large amount of functionality provided by a hospital CRM.
Ideally, the system will provide individual patient records with a list of available forms (that we will supply) that can be dragged and dropped into the patients file and then completed and saved. Staff will need individual log in and password to access.

Project will be offered in stages with total project budget up to $1500
