We have a design team at my company who use Adobe Acrobat Pro to design and create PDF forms. We do not need any assistance designing these forms.
These forms are printed by sales staff and provided to clients to fill-in manually, and scanned back up on to the system as image files. We are now looking to create interactive versions of the PDFs we have so that they can be filled in by our sales advisor on their desktops and signed using electronic signature pads. This will save the sales staff having to print and scan the forms.
So far we have been able to achieved the following: (As you will see from the attachment I).
1. Use Adobe Acrobat Pro to add interactive fields to the PDFs.
2. Use Adobe Acrobat Pro to make these field mandatory (always required)
3. Use Adobe Acrobat Pro to Add signature fields that can only be completed if the mandatory fields have been filed in.
4. Complete forms using software from Signotec called signosign/2. This software allows us insert signatures using electronic signature pads (this hardware is also provided by Signotec.
What we have not been able to achieve so far is to create fields that “conditionally required” I.e. fields that become mandatory / required fields depending on how earlier fields have been completed in the pdf form. When being completed electronically, we want the PDF document to have built in rules to ensure the form is fully complete befor being signed. Having robust controls built into the document would remove the risk field being omitted and save us time having to check the forms for completeness.
On the bottom right of page 1 of the attached doc there is a question “I confirm that I am working 8 hours or more per week. Please tick to confirm”. The answer to this question should only be mandatory / required if the previous answer (“Are you working as a job sharer?”) is yes. When being completed electronically, we want the PDF document to ensure this field is always completed if relevant. This would save it having to be checked, and remove the chance error.
Similarly, re medical question on page 3, the first line of the additional medical information should be required if you have answered yes to 1 (only) medical question, the first and second 2nd line should be required if you have answered yes to 2 only medical questions.
Initially We are looking for a proof of concept to show that it is possible to include the controls we require in the document. Thereafter we are looking to either train our staff on how to include these repeat scripts.
There after outsource the whole process of making the fields interactive to successful bidder. We have in the region of 300 Documents to prepare of the remainder of this year. This will be a separate job.
Additionally these forms change regularly, there will be ongoing assistance required
