Astrisk - FreePBX on ciso

For my VoIP, im using FreePBx on Cisco 7911.
If I create new extension, the phone is stuck on "registration"
Logs show that: Registration from '<sip:>' failed for '' - Wrong password
and if i check the phone status itself:
[login to view URL]
Error Updating Locale
even the locale show is right.
