Hi) First of all and standart option is to use php+mysql - but it could need to change server. Currently php8 is quite modern and fast and good known language. But Another option is to use some kind of Google Sheet+Google Forms + Google Apps Script, all depends on two things 1- does your form contain some very special things which couldnt be built in Google Forms 2- how is used final html page - can we build something similar in Google Apps Script etc. Must it be sent once somewhere or user can return to this page always or he must (maybe) receive email with pdf file and it will be good for you? Benefits of using Google Sheet - it can be partially customized even by people who dont know programming languages and can be somehow beautified etc. Also google sheet solution doesnt need server.
