A geo-stamped DCR only proves a visit if you decide what happens when a rep has no signal inside a clinic. Device time can be changed, so I'd queue offline and validate the timestamp on the server. I haven't shipped a native Android field app, but I wrote Firebase Cloud Functions for a mobile app built in FlutterFlow, so the sync backend is familiar ground.
