CureCaster sounds like a clean architectural problem: the runtime is only as good as the workflow JSON schema, and the agent is only as useful as the schema it has to populate. Tight schema (typed nodes, explicit transitions, clear decision payloads) and both sides become simple. Loose schema and the runtime grows edge cases while the agent hallucinates structure. That schema design is where I'd want to spend the first few hours. On the stack, React + TypeScript + AWS App Runner is the right call, and building on a HIPAA-eligible stack now (App Runner, RDS, Cognito, KMS) means swapping to PHI mode later is a config change, not a rewrite. For the agent, Claude Sonnet 4.6 with tool use and structured outputs gives the most reliable workflow-JSON generation, especially with prompt caching on the schema spec so each clinic interview doesn't repay token cost. Co-build suits me. Default workflow is Cursor + Claude Code for fast iteration, comfortable on pair-programming screen-shares, and I work well with a non-engineer founder driving product decisions.
