New Website required to sell ebook through [url removed, login to view]

I need a new website developed to be finished, tested and uploaded within 2 weeks.
The attached files are from a stalled work I commissioned. I want to recreate the design, colors and layout -- the word 'Cures' is now 'Treatments', and use the design work already produced, where appropriate. I also want to use a different photograph that I don't have to pay fees on (the one used here is from Getty).

. 4 ebook covers to a standard similar as the attached ebook file
. Effective layout of my wording content that I will provide to you -- copyrighting talent required. Experience with producing other Clickbank sites would be useful.
. Comparison with similar sites for legal advice to check my wording on appropriate disclaimer "the information on this site is not meant to convey medical advice - you must consult with your doctor etc'
. Insertion of webstats code and other php as required
. Fast loading
. Appropriate Meta tags, website security, etc
. Other search engine advice/Clickbank advice and any small unexpected extras that may have been overlooked in this spec.
I would like to see examples of similar websites that you have produced, please.
I would also value it if you are responsive, reliable, resourceful, work to a high standard, and display initiative eg show me early prototypes so we can be happy as early as possible with the design.
