I have provided 4 screenshots. I have added 1 cover page, so that people know clearly and instantly what the document is all about. A clickable Table of Index so that people can click the location they need and go directly to that particular page rather than scroll endlessly. And 2 sample inner pages, one for Houston and another for San Antonio. Since your document has many more entries, i would rather do the remaining once you like the idea. So, please let me know. if you like the basic idea, you can choose me the winner and I will do the remaining entries in the same format.
