Replace website with new theme, copy, photos and video

We have an existing website and have recently purchased a new theme.
We have already paid a copywriter to create the copy for each page of the new site (this is currently saved as a Word document).
We also have many (if not all) of the photos and video we want on the new site.
What we need is someone to help us pull it all together and create a professional (and effective) layout and look. We also need this person to have the technical skills to replace the old site with the new theme once it is complete.
I have attached our responses to a generic web design questionnaire that should provide a good understanding what what we're looking for.

Video is an important component of this website, both embedded videos and video playing in the background (no audio) on some of the pages. We were hoping to see more entries that had samples of websites with more video.

Entry #1 , There is a Film Section, It is playing a video without audio.. If the user click on it then he will listen the audio otherwise no audio..
