Contest Brief

I want the file attached below to be redesigned in a way that will make it look stylish, and corporate like a BILLION DOLLAR COMPANY form. The output file you'd submit has to be in Microsoft word. You can use the attached logo file where necessary.

PLEASE NOTE: I need to use this form urgently. Hence, I'd award the contest to the best designed form even before the end of the contest, if I see something that I really like.

The winner to whom I'd award this contest must have 5 star rating on the cover page and at least 4 star ratings on all the inside pages. So, it's important you submit the pages as separate entries so that I can rate them correctly. For example, if you submit a 5 star cover with a one star inside page as one combined entry, I'd award that entry as one star. Whereas, if you submitted them separately, I'd award the cover 5 star, and the inside pages one star. This way, you'd know that I am okay with the cover and you'd only need to work to make the inside pages 4-5 star to win the contest.

Most of you are having difficulty doing any thing good in terms of cover. I have just attached a front cover template sample that any of you could use by simply modifying the text, color and logo on it to come up with a better cover for this form. You'd see the attached file with file name 'LSG-3-final-Q.zip'

users are supposed to fill this form with their answers to the questions asked.For each question, you are supposed to leave a space under it where users are supposed to fill in their answers. Your form design is not okay without leaving the required space where the users are supposed to enter their own answers to each question.

I am not just interested in having a beautifully designed cover for the form, I am also interested in getting an elegantly designed inside pages and table. Every page of the form has to be designed excellently to WOW anyone that sees it.
