I need to have a form created in Excel like the one I am attaching. It needs to look professional as I will be using it for my customers. I need all of the fields to be editable so I can change them for different customers. I want the form to resemble the sample that has NORTHAMPTON HS.

I will need to know the size of the images for when I change them.

I need this as soon as possible.
