Hi,

I'm not too sure if this should be a contest for a project, but we will give it a go here.

I currently have a word document that I use as a form and fill out each time we use it. The issue is that it is out of alignment and is a pain to deal with when you have to fill it out.

What I am after is some form of template that I can open up each time we need to use it and then just fill in the blanks and not have to worry about deleting or moving any of the main form. Also I want to be able to do the tick boxes easier, currently we have to paste and copy each time we want to put a tick or X in one.

I'm not sure what program would be best to design this in, we currency have Microsoft office. I know there are programs you can download to iPad and similar that will let you create forms and fill them out onsite, but they are all subscription based. I want something that I can use without having to pay a monthly fee.

I have attached a copy of the form we currently use as well as the new logo that I would like you to use on the new form.

Hello Sir:
I'm an engineer with training on safe work proccedures. I'm also have experience in document templates and forms.
I'm curious about your requirements:
This form, it's for being filled in-site, right? I mean that you have to print, and then fill the printed paper? Or just it only needs to be filled in., let's say, an electronic way.
The last page require signatures (hand writen). How this can be solved?

  For now I'm happy with the old fashioned printing it out and taking it to site so we can fill it out.
  As you have noticed I do need to get those onsite to sign off on it. In the future I may have a form that I can do on the iPad whilst out onsite. But for this project all I need is this current form reproduced, made more user friendly and rearranged so that it looks better.
  
  Although if anyone can come up with a good system that I can fill in on the iPad and get signed off then I would consider it, although it may be a little out of my budget for now I'm guessing.
