With this form, I would like it to be more professional include my logo, have a copyright. Maybe it needs to be two pages. I would like form fields, that report to a spreadsheet in blue beam. I would also like images and links to the products in my store along the bottom of the page. This will be a form that I share with people in my industry in hopes that that will visit my store and purchase products. Maybe include the description that is on my homepage meta tag. I can provide images as necessary. Please provide samples of work. I have many more similar forms to be made.
