We need screens (Photoshop files) that show a modern state-of-the-art multiple-choice questionnaire.
(In a later project, we will animate this questionnaire and put it online.)

Format: Screen with 16:9 ratio, possibly hires.
You are quite free in chosing a design. The idea is "serious, but with a gaming touch".

The user will experience 4 topics; in each topic the user must select 2 answers from 8 given answers.
The selected answers must be graphically highlighted.

You will create at least 4 different basic patterns for a topic - see the very rough draft in the attached PDF:

1) All answers come in rectangles. Answers are text only.
2) All answers come in rectangles. Answers are text plus small size image (This is not included in the attached PDF, pls improvise)
3) All answers come in various geometrical objects. Answers are text only.
4) All answers come on pieces of a jig-saw puzzle. Answers are text only.

In the corner, pictographs should be included. You should put it into color.

The total number of topics(=4) and the numer of the current topic should somehow be represented. You may use a dotted progress bar or whatever seems cool for you.
There must be a NEXT and PREVIOUS link, they can well be graphical (arrows)!
