I require a Gift Voucher to supply to customers. It must be in A5 pdf file format but will have several fields that can be edited in Adobe Illustrator.

The fields include:

Name:
Sender:
Amount:
Gift Code:
Message: (make this bigger as the message may be a few sentences)

The voucher should also show our business name, SCINTEX. Web address scintex.com.au. Email: sales@scintex.com.au
And logo. See file attached.

Please keep in mind that Scintex is a company that sells industrial products, four wheel drive products etc.
Most of the Gift Vouchers so far do not suit this type of business. The products are purchase mainly by men. The colours should be silver and blue. Here is the website www.scintex.com.au
