I need to make 1 dynamic html form with questions.

I want it act exactly like in sample above
don't spent time on design, provide me just html

include "back", "forward", "continue", "end" links

it can be developed in any technic of: javascript, jQuery, html5

this is very simple work to whom who knows how to do it
