There needs to be a left and right section as listed in this template website.

  Looks good .. but your Date-Picker does not work on all bowsers.. Fails in IE

  I didn't do anything with the datepicker as you said you only wanted bootstrap markup. Jquery datepicker can be added easily with a single line of JavaScript and adding a class to the date inputs. Trivial to do.

I just want to confirm that this is for an HTML and Bootstrap project, only. The form(s) itself don't actually require functionality?
